A creature is allowed to make an Attack of Opportunity against a character it cannot normally reach if some part of the creature in question (or its equipment) must enter the character’s square. This Attack of Opportunity may only be made against the object or part of the creature that enters the square in question.

For example, creature with a natural 10 foot reach makes an Unarmed Attack against a fighter. The fighter may take the Attack of Opportunity, since clearly the creature’s hand enters the fighter square. Had the creature made a armed Sunder or Disarm attempt, the Fighter would be allowed to make attacks of opportunity only against the weapon (e.g. another disarm or sunder). Similarly, if a Medium sized creature makes an unarmed attack against a Tiny creature (with a reach of 2.5 feet or less), the Tiny creature is allowed to make the Attack of Opportunity, since the Medium creature’s arm clearly enters its square to make the attack.

(In the unlikely event of a water landing that it ever comes up, a Rogue is not usually permitted to make a Sneak Attack using this caveat, as the physical situation would not permit the Rogue to attack vital regions)
